The Department of Pathology and Clinical Laboratories (DPCL) conducts routine hospital services and support research activities based on the requirements of ISO15189. The number of samples examined in the department in 2021 is listed in Table 1. We conducted the following pathological diagnoses this year: 13,797 biopsy specimens, 3,398 intraoperative tissue specimens, 5,029 cytology specimens, and 7 autopsy cases.

Clinicopathological conferences are held regularly with each clinical department/section. Staff members participate in expert panel meetings to medically interpret the results of cancer gene panel tests every Thursday.

The Team and What We Do

Weekly meetings are regularly held with residents to discuss clinical questions or hypotheses inspired by pathology specimens and clinical information. The results are then published in papers. Some of the staff members also work at the EPOC, where they are engaged in research aimed at understanding the biological characteristics of cancer and developing novel therapies using in vitro systems, in addition to clinicopathological investigations.

As a biobank project, we are contributing to research activities by cryopreserving surgical specimens and producing tissue microarrays (TMA). In addition, in support of the TITANIA study, we are cooperating in the collection of specimens.

Research activities

The following are this year's major research results.

1. We clinicopathologically demonstrated that necrosis is a poor prognostic factor in lung adenocarcinoma.

2. We investigated the immune microenvironment of preoperatively treated lung cancer and found that the number of CD204-positive TAMs correlated with the amount of residual cancer cells and prognosis.

3. We found that the degree of necrosis in lung neuroendocrine carcinoma correlated with prognosis.

4. We investigated the immune microenvironment in preoperatively treated pancreatic cancer and found that the number of CD204-positive TAMs was an independent poor prognostic factor.

5. We found that drug-exposed cancer-associated fibroblasts (CAFs) promote the invasion of drug-exposed gastric cancer cells.

6. We explored whether extranodal invasion is a poor prognostic factor in surgically resected lymph node-positive lung adenocarcinoma.

7. Clinicopathologic, transcriptomic, and immunologic microenvironmental analyses indicated that hepatocellular carcinoma with a sarcomatoid component is a characteristic histologic subtype of hepatocellular carcinoma

8. In renal cell carcinoma, the presence of dirty necrosis strongly correlated with postoperative prognosis.

9. Compared to podoplanin-negative CAFs, podoplanin-positive CAFs secrete higher amounts of TGF-b and may influence the immune microenvironment in squamous cell carcinoma of the lung.

10. We contributed to the industry-university collaborative national genome screening project SCRUM-Japan using histopathological specimens.

Clinical trials

Pathological testing strengthens the quality control and quality assurance of clinical tests performed in NCCHE's clinical trials.


The staff of the Department of Pathology and Clinical Laboratories provide guidance to NCCHE residents regarding diagnostic training and training in pathological research. They also provide guidance to NCCHE doctoral students at Juntendo University regarding preparing their doctoral dissertations. Some of the staff members have supervised master's students as faculty members of the Graduate School of Frontier Sciences at the University of Tokyo.

Future prospects

In 2022, we will acquire CAP (College of American Pathologists) certification in order to contribute even more to the promotion of clinical research and to further improve patient care in accordance with the "Patient First" philosophy.

We will further promote translational research on cancer diagnosis and treatment.
