Introduction
Research is conducted; aiming to construct evidence connected to the development of cancer prevention by clarifying the causes of cancer in humans via the study base of a large-scale cohort study and others of local residents.
Research activities
1. Japan Public Health Center-based prospective study (JPHC study) / JPHC study for the NEXT generation (JPHC-NEXT)
Follow-up surveys and data analysis of the Japan Public Health Center-based prospective study (JPHC study) with 140,000 local residents as subjects have been conducted on an ongoing basis since 1990. In this study, we reported an increase in thyroid cancer risk for increased height observed in men (HR per 5-cm increase in height was 1.12 (95% CI 1.06-1.18)). We also found that an increase in BMI during adulthood was linked with a decreased risk of esophageal squamous-cell carcinoma (ESCC), with each 1% increase in BMI per 5 years corresponding to a 15% decrease in ESCC risk (95% confidence interval 9-21%). Additionally, we confirmed that smoking and alcohol intake were associated with an increased risk of oral cavity and pharyngeal cancer. Furthermore, we reported that dietary acrylamide intake was not associated with the risk of breast, endometrial or ovarian cancer. The baseline survey for the JPHC study for the NEXT generation (JPHC-NEXT) completed by the end of 2016 and the 5-year follow-up survey has been commenced since 2016 (Table 1).
2. Molecular epidemiologic studies to investigate the cause of cancer
We conducted a case-cohort study based on 33,736 JPHC study participants who responded to the baseline questionnaire survey and provided blood samples. In 3,301 incident cases of cancer and 4,044 randomly selected subcohort participants, we observed that higher plasma concentrations of vitamin D were inversely associated with the total cancer risk (P for trend=0.001). We also conducted a nested casecontrol study within the JPHC study. In 170 newly diagnosed pancreatic cancer cases and 340 matched controls, we observed that higher plasma concentrations of total branched-chain amino acids were positively associated with the risk of pancreatic cancer (P for trend=0.02).
3. Consortium on Large-scale Molecular Epidemiologic Studies in Japan
We have established a consortium on large-scale molecular epidemiologic studies in Japan, J-CGE (Japanese Consortium of Genetic Epidemiology studies). This consortium comprises four large-scale studies: the Japan Public Health Center-based Prospective Study (JPHC Study); the Japan Multi Institutional Collaborative Cohort Study (J-MICC Study); the To hoku Medical Megabank Project (TMM); and the Hospital based Epidemiologic Research Program at Aichi Cancer Center (HERPACC). Within this consortium, we are performing genome-wide association studies on height, weight, body mass index, and alcohol consumption stratified by ALDH2 variants. Further, to promote the collaborative research system, we built a platform within the Center for Public Health Sciences, National Cancer Center that enables remote access to the database and supercomputers at the Tohoku University.

Supervised the research of four graduate students and one research resident fellowship.
Future prospects
While focusing on the cohort structure of the JPHC study for the next generation (JPHC-NEXT) that becomes the study base, we hope to help cancer prevention progress by analyzing information and samples of existing epidemiologic studies by identifying new risk factors and the continued evaluation of risks in Japanese people.
