Introduction

 The Division of Health Care Research was established in January 2016 by the National Cancer Center to acknowledge the numerous individuals surviving cancer for extended periods and reflecting the need to study their unmet healthcare needs. Our division works to enhance the quality and longevity of all persons diagnosed with cancer and minimize or stabilize the adverse effects experienced during their remaining lifetime. Our division supports clinical research that examines the effectiveness of supportive, palliative and psychosocial care among cancer survivors and their families.

The Team and What We Do

 We have two teams, i.e. one for original research team and one for clinical research support. Our division focuses on epidemiological, interventional and behavioral and public health translational research in cancer survivorship to enhance survival and psychosocial wellbeing among all persons diagnosed with cancer and their family caregivers. Our division initiates an interdisciplinary research program ranging from basic bio-behavioral science to the development, testing and delivery of psychosocial, behavioral and survivorship care and also manages J-SUPPORT as a central support office.

Research activities

1. Health promotion research

 A survey was conducted to determine the extent to which recommendations of breast cancer guidelines for physical activity are recognized and practiced by medical professionals engaged in breast cancer care. We developed high-intensity, intermittent training (habit-B) using own body weight for sedentary breast cancer survivors. The blood composition of alpha-linolenic acid and gut bacterial diversity was inversely associated with the fear of cancer recurrence, respectively in breast cancer survivors. In the meta-analysis, we also revealed that improvement in anxiety symptoms was associated with omega-3 polyunsaturated fatty acid treatment compared with controls.

2. Patient-physician communication research

 We reported that as a communication method for prognostic discussion with high uncertainties, the best way to communicate news, both good and bad, is to discuss with the cancer patients clearly and optimistically, taking goals and continuity into account. We reported that patients with task-thinking coping styles required sympathetic responses. Explicit prognostic disclosure prompted better outcomes than non-disclosure in Japanese women with breast cancer. When asked about the prognosis by Asian patients with cancer, clinicians may be encouraged to respect their wishes and explicitly discuss the prognosis if deemed appropriate.

3. Suicide prevention research

 To investigate the suicide risk among cancer patients in Japan and identify the patient subgroups and time periods associated with higher risk, we developed a protocol for the nationwide population-based study using data from the National Cancer Registry and developed materials to encourage mental health care access for cancer patients.

4. Developing cancer survivorship guidelines

 We identified English and Japanese guidelines including cancer survivorship in their titles and proposed the need for guidelines on optimal coordination of care between oncologists and other healthcare providers to support patients along the cancer care continuum and specifically to encourage a healthy lifestyle as part of cancer survivorship. We also organized the Cancer Survivorship Guidelines General Committee and concluded an agreement on important clinical issues included in the guidelines

5. Supportive care clinical research

 In consultation with medical device manufacturers regarding cooling therapy to reduce chemotherapy-induced peripheral neuropathy, work to develop a circulating cooling device got underway.

Future prospects

 We are planning to start evaluation research with provisions to establish cancer survivorship guidelines in mind. We plan to promote behavioral research to bridge the evidencepractice gap in the field of cancer survivorship and we will continue to provide consultation on research design and statistical analysis and support clinical trials in terms of quality management.
