Introduction
Research is conducted with the aim of constructing evidence connected to the development of cancer prevention by clarifying the causes of cancer in humans via the study base of a large-scale cohort study and other studies of local residents.
Research activities
1. Japan Public Health Center-based prospective study (JPHC study) / JPHC study for the Next Generation (JPHC-NEXT)
Follow-up surveys and data analysis of the JPHC study with 140,000 local residents as subjects have been conducted on an ongoing basis since 1990. As notable findings during the 2019 fiscal year, we reported that intake of fermented soy products was inversely associated with all-cause mortality in both sexes (highest versus lowest fifth group: hazard ratio [HR] = 0.90, 95% confidence interval [CI] = 0.83 to 0.97, P for trend = 0.05 in men, and HR = 0.89, 95% CI = 0.80 to 0.98, P for trend = 0.01 in women). Additionally, we participated in pooled analysis from 26 prospective studies and evaluated associations of cigarette smoking and alcohol consumption with biliary tract cancer risk. This pooled analysis showed being a current smoker was associated with increased extrahepatic bile duct cancer risk (current smokers versus people who had never smoked HR = 1.69, 95% CI = 1.34 to 2.13). This result was similar to our previous report from the JPHC study. The baseline survey for the JPHC-NEXT completed by the end of 2016, and we started the 5-year follow-up survey in 2016 (Table 1).
2. Molecular epidemiologic studies to investigate the cause of cancer
Within the JPHC Study, we conducted a case-cohort study with 10,536 subcohort subjects and 3,541 newly diagnosed cancer cases derived from 32,949 eligible participants aged 40-69 years. By using Mendelian randomization analyses of 29 known type 2 diabetes susceptibility variants, we estimated the HRs per doubling of the probability of diabetes were 1.03 (95% CI = 0.92 to 1.15) for total cancer risk. Our findings suggest that there is little evidence to support the genetic role of type 2 diabetes in cancer development in the Japanese population.
3. Consortium on Large-scale Molecular Epidemiologic Studies in Japan
We have established a consortium on large-scale molecular epidemiologic studies in Japan, J-CGE (Japanese Consortium of Genetic Epidemiology studies). This consortium consists of four large-scale studies: the JPHC Study; the Japan Multi-Institutional Collaborative Cohort Study (J-MICC Study); the Tohoku Medical Megabank Project (TMM); and the Hospital-based Epidemiologic Research Program at Aichi Cancer Center (HERPACC). Using the results of genome-wide association studies on Body Mass Index (BMI) in this consortium, we have performed analyses regarding BMI and colorectal cancer using the method called Mendelian Randomization analysis. Moreover, we have been performing Mendelian Randomization analysis regarding glycol-metabolism markers such as Hemoglobin A1c, lipid-metabolism markers and Vitamin D, and colorectal or total cancer incidence.
Education
Supervised the research of four graduate students and one research resident fellowship.
Future prospects
We would like to construct cohort studies for understanding risk factors for a broad range of diseases, taking into consideration changes in social environment and needs. By analyzing data from the cohort studies, we hope to contribute to developing evidence for personalized cancer prevention and promoting a lifetime of health.
